A cardiac risk profile health screening will be held Dec. 15 with a chance to win an incentive prize.

Assessments will take place in Bldg. 3705's aerobics room. Screenings are free annually for DOD civilian employees.

Participants need to complete a Health Risk Appraisal (HRA) before making an appointment to collect a prize. Go to www.afmcwellness.com to fill out an HRA. Please bring a copy of the HRA certificate and your motivational user ID to the screening.

To schedule an appointment, call Sports Director Pat Griffin at 734-4664. Appointments can be made from 7:30 a.m. to 10:15. Walk-ins are welcome, but those who make appointments will have priority.

For questions, contact CHPS at 582-6817 or email CHPSTinker@psc.gov.

Fasting eight to 12 hours before the appointment is preferred. The tests will measure total cholesterol with a lipid panel, blood glucose, blood pressure and a body mass index calculation.
