Lost ID? Follow these steps to get a new one

Have you ever felt the panic of not knowing where your base ID card is?

Unfortunately, that feeling has been shared by many on base. If you're one of them, remember there are steps to follow to report a lost or stolen ID (military, military retiree, dependent, civilian or civilian retiree) card.

If you suspect your ID has been stolen off base, you first need to fill out a police report with off-base authorities. Your next stop is at Pass and ID in Bldg. 6611, just outside the Tinker Gate. There, you will fill out an AF Form 1168, Statement of Suspect/Witness/ Complainant. You will also be verified through the Defense Eligibility Enrollment Reporting System, or DEERS Office, during normal duty hours, at which point you will be given a base access pass to go to the ID card office in Bldg. 460 to obtain a new card. You must take a copy of the AF Form 1168 with you to Bldg. 460 before a new ID card will be issued or you will be turned away.

"Everyone who has a base ID card is registered in DEERS and can be verified as authorized to have said credential," said Dallas Stroud, 72nd Security Forces Squadron. "It is the only way that the ID cards are issued."

If your ID was stolen on base, you must first fill out a report at the Law Enforcement desk, located in Bldg. 591. There, you will fill out an AF Form 1168 and take a copy to Bldg. 460 for a new ID card.

If your ID was lost on or off base, you must first stop in Pass and ID before heading to Bldg. 460 for a new ID card. You will fill out AF Form 1168 and take a copy to Bldg. 460 for a new ID card.

If a DBIDS Contractor Identification Card (issued by Pass and Registration) was lost or stolen, there are a few extra steps to obtain a new one. 

If the credential was stolen on or off base, you must fill out a police report with the on or off base police agency. You will stop at Pass and ID with a new AFMC Form 496 along with a copy of the police report (or 1168) to obtain a new DBIDS contractor Identification card. 

If the credential was lost, willfully damaged or not surrendered to Pass and Registration (including seizure at the gate for expiration), a $50 charge will be assessed before a new credential will be issued.  Personnel will need to be sponsored on and report to the Accounting and Finance, Accounts Receivable Unit Office. There, you will pay the $50 charge and fill out a DD Form 1131 (Cash Collection Voucher). The voucher will be hand carried to Pass and ID along with a new AFMC Form 496 prior to issuing a replacement DBIDS ID Card. 

Master Sgt. Justin Walker, 72nd Security Forces Squadron, said a lot of people who have lost their ID cards have a family member or friend bring them on base and then they go straight to the ID card office in Bldg. 460. But that's not the way to accomplish their goal, he said.

"Active duty military and federal civilian employees who have had their ID lost or stolen are required to provide a letter from their commander or director prior to getting a replacement," said Staff Sgt. Jacinta Johnson, 72nd Force Support Squadron.
"If they do not have the AF Form 1168 in hand, the ID card office will send them right back to Pass and ID," she said. "They must fill the paperwork at Pass and ID first, so this will save them a step."

If an ID card is lost or stolen after business hours, Mr. Stroud says you'll need to get someone to sponsor you on base until the office opens. This would mostly apply to a person who lives on base, otherwise most people would wait until their next duty day to report.
