Hazard reporting program detects unsafe, unhealthy conditions early Published Feb. 23, 2008 By Steve Serrette 72nd ABW Safety Office TINKER AIR FORCE BASE -- Detecting unsafe or unhealthful conditions at the earliest possible time and abating hazards promptly at the lowest possible working level are essential elements of the Air Force Occupational Safety and Health program. Mishap prevention depends on personnel identifying, reporting and correcting hazards promptly and efficiently. The Air Force has a formal hazard reporting program using the Air Force Form 457, USAF Hazard Report and any person assigned, attached or under contract to the Air Force may report a hazard. The AF Form 457 hazard reporting program provides a system of reporting hazardous conditions and for proper investigating and correcting of those hazards. Personnel can submit a hazard report using the AF Form 457 on any event or condition that affects flight, ground, weapons, or space safety. Reportable hazards include unsafe procedures, practices, and/or conditions. Always give your supervisor (the lowest possible working level) the first chance to eliminate the hazard. If the hazard is eliminated on the spot, no further action is required. If the hazard presents imminent danger, the supervisor must take immediate action to correct the situation or apply interim control measures. Personnel can report hazards that can't or won't be corrected at lower levels by mail to the 72nd ABW Safety Office via the AF Form 457 or via email to the 72nd ABW/SE Workflow box. Personnel can also report by telephone at (455) 739-3263 or in person at building 1, suite 100B. Reports may also be submitted anonymously. Tenant personnel should forward Hazard Reports through their assigned Safety Office, which in-turn will be routed to the 72nd ABW Safety Office. The Safety staff will investigate the hazard report; discuss the hazard with the submitter, responsible supervisor or manager and other applicable parties to determine if the hazard is valid and what interim controls and corrective actions are required. Initial response will be made within ten workdays. If personnel submit their contact information on the form, then they will receive a reply from the Chief of Safety indicating proposed corrective actions and assigned OPR. If the hazard report is determined to be invalid, rationale for that determination will be furnished to the submitter.
